1.身份证件：身份证或护照原件。

5.1既往疫苗原始接种记录原件及复印件一份（中文/英文）。
5.2已持有《疫苗接种或预防措施国际证书》请携带原件。
5.3若有姓名更改记录，请携带户口簿原件及复印件（更改姓名页）。
6.经批准出国劳务、留学、探亲、定居及其他出境1年以上的中国公民：如满足以下条件可申请办理传染病监测免征体检，现场审核须提供相关证明材料原件和复印件一套：
6.1劳务：有效护照、工作签证、劳务公司出具的证明。
6.2留学：有效护照、留学签证、境外学校出具的录取通知书等证明文件。
6.2.1留学申请说明：
①申请须在录取通知书或邀请函标明的开学日期前进行；
②留学签证有效时间≥365天（闰年366天）；
③赴美留学须携带I20表或DS2019表，表中“PROGRAM START/END DATE”或“Form Covers Period”时间≥365天（闰年366天）；
④留学基金委公派留学生须携带《国家留学基金资助出国留学资格证书》，其中注明的“留学期限”和“资助期限”均≥12个月。
6.3探亲、定居：有效护照、探亲类签证、目的地国家出具的邀请函和机票。
6.4船员：海员证、有效《中华人民共和国海船船员健康证书》及详细船员健康体检报告（体检报告复印件2份）；或海员证、相关国家的国际海员签证和其他有效身份和签证等证明文件及所在单位出具的证明文件。
6.5赴澳大利亚体检：持有澳大利亚使馆发放的含有HAP号码的体检者在定点签证板块进行预约。其余体检者，请在出境板块预约。注意：免征传染病监测体检项目仅限《国际旅行健康检查证明书》内容，其他国家额外要求的体检项目不予免征。 以上未尽事宜以我中心最终解释为准。
1.体检当日空腹（禁食8小时以上，可少量饮白开水）。
2.体检/预防接种前2-3天，应避免过度疲劳，忌暴饮暴食及饮酒。
3.未满18岁儿童，须监护人陪同办理。
4.正在发热及患急性病的申请人，建议暂缓进行体检及预防接种。
5.需长期服用药物者（如高血压患者），体检当日正常服药。
6.16周岁以上人员均要求进行X光检查。不满16周岁、妊娠0-3个月者（须提供妊娠证明）可选用PPD/IGRA 替代，若PPD或IGRA 检测结果为阳性则仍须进行X光检查。
7.女性体检者请尽量避开月经期及前后3天。
8.孕妇请主动告知并携带孕检证明。
9.根据《中华人民共和国国境卫生检疫法》及其实施细则，您在本中心接受传染病监测体检项目包括：临床检查（含一般健康检查、放射科、心电图、超声等) 、血液检查（含乙肝表面抗原、丙型肝炎抗体、人免疫缺陷病毒抗体、梅毒螺旋抗体特异抗体、血常规、血生化等）、尿常规。由于体检结果涉及个人健康隐私，请在领取体检报告后务必亲自妥善保管。
10.体检时，应着上下分身衣服。进行胸部X线检查时，为保证拍片质量，上身应着薄棉质地的衣服，冬季可着贴身秋衣。衣服表面均不能带有任何珠子、扣子、丝线、刺绣、胶印图案等装饰物。女士脱去内衣(可穿无钢托运动内衣)，扎起长发，颈部不宜戴任何饰物。
11.体检结果异常者，本中心将及时与受检者本人联系，受检者应在要求时间内携带本人有效身份证明原件到原体检地点进行复检。
1 .结核菌素皮肤试验（PPD） 方式：皮内注射 办理地点：和平里本部 办理时间：工作日周一、二、三、五（法定节假日前两天不可做） 上午8:00—11:00；下午13:00—15:30 结果读取：须本人48-72小时内到本中心读取结果。
2. 全血干扰素释放试验（IGRA） 方式：抽静脉血 办理地点：和平里本部、海淀门诊部 办理时间： 和平里本部工作日周一至周五上午8:00—11:00（法定节假日前一天不可做） 海淀门诊部工作日周一至周五上午8:30—11:00（法定节假日前一天不可做） 结果领取：四个工作日（可与体检结果同取），不可加急。
1.领取时间：自体检/预防接种转录之日起第四个工作日后领取（仅限工作日下午13：00-16:00）。如需办理加急，请向登记台工作人员出示4日内离境机票或行程单。
2.领取方式： （1）现场取证，持取证凭证领取。遗失取证凭证者须携带体检者本人身份证件原件及复印件。 （2）快递服务，可现场办理寄送业务，体检/疫苗转录证书将于办理日四个工作日后送达。
申请来华定居，或任职、就业、学习在华居留1年或1年以上的外国人。
1.身份证件：护照原件。
2.证件照片：近六个月内拍摄的白底小两寸彩色证件照4张（4.8×3.3cm）（请预先剪裁好）。
3.首次来华任职、就业、学习在华居留1年或1年以上的外籍人员，如有以下相关证明材料，请携带（原件和复印件1份）并主动出示，如没有请忽略：
3.1办理工作居留：护照（含Z或R签证页）、《中华人民共和国外国人工作许可通知》。
3.2办理留学居留：护照（含X1签证页）、录取通知书、《外国留学人员来华签证申请表》（JW201/202）。
4.其他情况办理学习类居留：请携带并主动出示至少一项以下材料：
4.1教育委员会出具的同意其就学的函件复印件；
4.2《录取（入学）通知书》复印件；
4.3《外国留学人员来华签证申请表》（JW201/202）复印件；
4.4护照姓名页和X类签证页复印件。
1.请在入境后尽快办理健康体检手续，以免影响后续居留许可的办理。
2.体检前需禁食6小时以上，可少量饮用白开水。

4.验证材料 （1）《外国人体格检查记录》表所体现的内容应由中文或英文填写，签发日期应清晰明了,此表有效期从签发之日起六个月内有效。此表须包括受检者照片（加盖医院公章）、体检结论、医生签名、医院公章（同照片处公章一致）。 （2）体检项目按《外国人体格检查记录》表的内容进行，并附上X光报告（或X光片）、心电图图纸或报告、实验室检测报告（至少包含HIV、梅毒检测）、超声检查报告等原始报告或者化验单。 （3）体格检查及体检报告的签发必须由当地的公立医院完成；如在私立医院或诊所完成，体检报告应先在当地进行公正。 （4）体检报告内容如须通过中国驻外国领（使）馆的认证的，申请人应向中心工作人员提供使馆的认证证明或者认证标签。 （5）如无中国驻外国领（使）馆的认证证明或认证标签的，或提交的“体格检查记录表”内容有缺项漏项、无原始报告单或报告显示不清晰项的，现场医师会根据实际情况补检相应的项目以达到验证所需要求。提交的《外国人体格检查记录》表无效的须重新办理健康体检。
5.证书领取 1.领取时间：办理之日起第四个工作日后领取（仅限工作日下午13：00-16:00）。如需办理加急，请向登记台工作人员说明情况。 2.领取方式： （1）现场取证，持取证凭证领取。遗失取证凭证者须携带体检者本人身份证件原件及复印件。 （2）快递服务，可现场办理寄送业务，证书将于办理日四个工作日后送达。

Foreigners who apply to settle down in China, or hold a post, obtain employment or study in China for one year or more.
【Materials should be carried】
1.Identity document: Original passport.

3.Foreign nationals who reside in China for one year or more for the first time to take up posts, work, or study in China, if any,please bring the following relevant supporting documents (original and one copy) and present them proactively. 4.Other situations apply for study residence: Please bring and actively present at least one of the following materials 4.1 Copy of the letter from the Education Committee approving their enrollment; 4.2 Copy of Admission Notice; 4.3 Copy of "Application Form for Foreign Students' Visa to China" (JW201/202); 4.4 Copy of passport name page and X-visa page.
【matters needing attention】
1.Please complete the health examination procedures as soon as possible after entering the country to avoid affecting the subsequent processing of the residence permit.
2.Before the physical examination, it is necessary to fast for more than 6 hours and drink a small amount of plain water.
3.During the physical examination, one should wear clothes that are split up and down. When conducting a chest X-ray examination, to ensure the quality of the film, thin cotton clothing should be worn on the upper body, and close fitting autumn clothes can be worn in winter. The surface of the clothes must not have any decorative elements such as beads, buttons, silk threads, embroidery, offset printing patterns, etc. Women should take off their underwear (can wear non steel sports underwear), tie up their long hair, and avoid wearing any accessories on the neck.
4.Minors under the age of 18 must be accompanied by a guardian when processing.
5.Applicants who need to take medication for a long time (such as hypertensive patients) should take medication normally on the day of the physical examination to avoid fluctuations in their condition.
6.Applicants who are suffering from fever or acute illnesses are advised to postpone the medical examination.
7.Ladies, please check whether you are pregnant or not, and pregnant women, please bring your “pregnancy test certificate/record” and show it to the receptionist on your own initiative.
【Notice on Verification of Overseas Physical Examination for Foreign Guests】
1.Holding reservation SMS message.
2.Identity documents: original passport.
3.Two small two-inch color ID photos (4.8×3.3cm) with white background taken within the last six months (please pre-cut them).
4.Verification materials: (1) The contents of the Physical Examination Record For Foreigners should be filled in Chinese or English, and the date of issuance should be clear. This form is valid for six months from the date of issuance. This form must include a photo of the examinee (stamped with the hospital seal), the medical examination conclusion, the doctor's signature, and the hospital seal (consistent with the seal in the photo). (2) The physical examination items shall be carried out according to the contents of the Physical Examination Record For Foreigners, and the original reports or test sheets such as X-ray reports (or X-ray films), ECG drawings or reports, laboratory test reports (at least including HIV and syphilis tests), ultrasonic examination reports, etc. shall be attached. (3) The medical examination and the issuance of the medical report must be completed by a local public hospital; if completed in a private hospital or clinic, the medical report should first be impartialized locally. (4) If the content of the medical report has to be certified by the Chinese consulate (embassy) in the foreign country, the applicant should provide the staff of the center with the certificate of certification or certification label of the embassy. (5) If there is no certification certificate or certification label from the Chinese consulate (embassy) in a foreign country, or if there are missing items in the “Physical Examination Record For Foreigners”, or if there is no original report card or the report shows unclear items, the on-site physician will conduct additional tests on the corresponding items according to the actual situation to meet the verification requirements. If the Physical Examination Record For Foreigners submitted is invalid, a new physical examination must be carried out.
【Certificate Collection】
1.Collection time: after the fourth working day from the date of processing (13:00-16:00 pm on working days only). For expedited processing, please explain the situation to the registration desk staff.
2.Collection method: (1) Collect on-site with a collection voucher. Those who have lost their collection vouchers must bring the original and a copy of the medical examiner's own identification document. (2) Express delivery service, on-site delivery service is available, and the certificate will be delivered four working days after the processing date.
Notice: Because there may be some technical and policy differences, acceptance of the physical examination record should be depended on our consulting doctors. You may be required to take some tests for the suspicious results or items you didn’t finish abroad in order to complete the confirmation. Thank you for your cooperation!
（1）已知对本疫苗的任何组分，包括辅料成分以及鸡蛋过敏者。
（2）患急性疾病、严重慢性疾病、慢性疾病的急性发作期和发热者。
（3）妊娠期妇女，正在哺乳9月龄以下婴儿的妇女。
（4）免疫缺陷、免疫功能低下或正在接受免疫抑制治疗者。
（5）患脑病、未控制的癫痫和其他进行性神经系统疾病者。
（6）6月龄以下婴儿。
（1）以下情况者慎用：家族和个人有惊厥史者、患慢性疾病者、有癫痫史者、过敏体质者、哺乳期妇女。
（2）由于存在疫苗相关的嗜神经性疾病发生风险，不建议6-8月龄婴儿接种，但疾病流行期间黄热病毒传播风险非常高时除外。
（3）60岁以上老人发生严重全身性不良反应的风险增加，应密切关注疫苗相关的嗜内脏性疾病的发生风险。
（4）接种本疫苗和注射免疫球蛋白应至少间隔3个月以上，以免影响免疫效果。
（5）使用其他减毒活疫苗与接种本疫苗间隔至少一个月。
黄热病疫苗属于减毒活疫苗，存在一定接种风险。为更好地了解您的身体状况，最大限度减少和防控接种风险，我中心建议您在接种黄热病疫苗前进行评估体检。体检当日请空腹（禁食8小时以上，可少量饮白开水），体检前2-3天应避免过度疲劳，忌暴饮暴食及饮酒，女性体检者请尽量避开月经期及前后3天。
若您近期在医疗机构进行过体检，请携带半年内的体检报告，报告需包含但不限于如下项目：内外科、腹部B超、胸部X光、血常规、肝功能两项（ALT、AST），肾功能三项（肌酐、尿素、尿酸）、尿常规、艾滋病抗体。未包含的项目可以在中心进行补项检测。
1、免费接种：应满足以下要求之一
①凭前往世卫组织公布的疫区国家或地区的签证和机票。
②因免签、过境免签、落地签证等入境签证政策而无法提供签证的情形，凭前往世卫组织公布的疫区国家或地区的机票。
③持入境时海关出具的《黄热病预防控制告知书》以及本人有效的护照或其他入境证件，经评估符合接种条件的。
2、收费接种：其他申请接种黄热病疫苗、不符合免费接种政策的，参照《中华人民共和国疫苗管理法》非免疫规划疫苗相关规定收取费用（185元/剂）。
1、本人有效身份证或护照原件；

5、18周岁以下未成年人须有一名法定监护人陪同，监护人需携带本人有效身份证或护照原件。
